CHAMP Athlete of the Month

Purpose

a. To recognize or highlight one local youth athlete each month for exemplifying outstanding character in the athletic world.

Nomination Process

b. Anyone can nominate a student-athlete for the CAM award. Club and school coaches, parents, teammates, and et cetera will fill out a CHAMP CAM form to nominate a student athlete for recognition by CHAMP as an athlete of character.

i. What to be included:

  1. Athlete Name
  2. Age
  3. Contact Information (phone, email, address)
  4. Explanation as to why the athlete should be considered for CAM award
    “I think Brittany should be nominated for the CAM award because…”
  5. Highly recommended: a story/description to support/illustrate the nomination.
  6. Nomination person’s name and contact information for validation and notification purposes.

ii. Send to CHAMP at:

  2. P.O. Box 272176, Fort Collins, CO 80527

Deadline

c. CAM nomination will be accepted at any time. CHAMP will choose a CAM recipient each month or preceding months based on the criteria and qualification process.

Evaluation

d. Two members of the CHAMP Team will review the story and determine whether or not the athletes nominated should be recognized as the CHAMP Character Athlete of the Month based on CHAMP values.

e. If CHAMP receives several nominations, they will be archived and saved so that an athlete would have the opportunity to be recognized as the Character Athlete of the Month (CAM) at a later date.

Post Evaluation

After evaluation, the Program Coordinator will contact the recipient and have them send CHAMP a photo, and answer the following questions for a Character Athlete of the Month Profile:

i. Age
ii. Sport
iii. School
iv. Parents
v. Siblings/Ages
vi. Interests/Hobbies/Activities
vii. Favorite athlete they admire
viii. Favorite sports team
ix. Quote from the person nominating the athlete, focused on why they thought the athlete should be nominated for a character based award.

After all of the information is collected, the Program Coordinator will develop a one page document that profiles the Character Athlete of the Month.

Character Athlete of the Month (CAM) Announcement

f. The recipients profile will be:

i. Featured on the website
ii. Announced/featured on Face book
iii. Announced on Twitter
iv. Sent out via Constant Contact to the entire CHAMP mailing base
v. Sent to NoCo5 for TV possible recognition
vi. Recognized in the program at the Annual CHAMP Community Breakfast

Award Process

g. The CAM will receive:

i. $30 Gift Certificate to Texas Roadhouse
ii. CHAMP Recognition Certificate
iii. Printed out CAM Profile
